Basic Consent and Release

The following is a basic Concent and release form.  It is not copyright and can be used by anyone.  A word of advice, whenever anyone gives you a release to sign, double check on the time constraints for the release.

Some clients are paying you for unlimited release of your work or likeness for “perpetuity,” which means forever.  If your work is union, there are usually set dates and amounts you will be paid.  And, in some cases residuals depending on how many times and where your likeness or work is seen.

If you can, try to read the release and any contracts before you start work.  If you have questions, be sure to ask your agent or the client.   Sometimes they are using a standard form and may be willing to adjust it.
